Support Coordination

Help to implement your plan

Assistance to strengthen participant’s abilities to connect to and coordinate informal, mainstream and funded supports in a complex service delivery environment. This includes resolving points of crisis, developing capacity and resilience in a participant's network and coordinating supports from a range of sources.


Training In Planning and Plan Management 

Learn to manage your plan

Training in planning and plan management for participants unable to do this independently but who could do all or part of the task with training.


Life Transition Planning 

Help when life changes

Establishing volunteer assistance within the participant’s home or community to develop skills. For instance, assistance in attending appointments, shopping, bill paying, taking part in social activities and maintaining contact with others.


AFA Support Coordination is a Registered NDIS Provider and able to provide the above supports to agency, plan and self managed participants.
